• Episode 186: Your Revenue Cycle Is Talking — Are You Listening?
    Jun 16 2026

    Send us Fan Mail

If I walked into your organization today and asked you one simple question — “What is your revenue cycle trying to tell you?” — would you know the answer?

Not your collections rate. Not your days in A/R. Not your monthly deposits. I mean the deeper answer. What operational stories are hiding inside your numbers right now?

Because most healthcare organizations still think about revenue cycle as a billing department function. Claims go out. Payments come in. Denials get worked. Aging gets reviewed. End of story.

But high-performing organizations understand something very different: your revenue cycle is not just billing. It is operational intelligence. It is one of the clearest mirrors of how your organization is actually functioning.

Every metric tells a story. Your denial patterns tell stories. Your charge lag tells stories. Your payer behavior tells stories. Your provider coding and undercoding variation tells stories. Your scheduling data tells stories. Even your unapplied credits and unpostables tell stories. And the organizations that thrive financially are usually the organizations that have learned how to listen carefully.

So today I want to challenge you to think differently about your revenue cycle. Not as a back-office function. Not as an administrative burden. But as one of the most important diagnostic tools in your entire organization.

Because here’s the truth: your revenue cycle is talking to you every single day. The question is whether anyone is listening.

    Jun 9 2026

Last week, we talked about some of the biggest differences between struggling medical groups and high-performing organizations. We discussed long-term thinking, strategic staffing, operational intelligence, investment philosophy, and diversified revenue streams.

And today, we’re continuing that conversation — because once organizations begin growing successfully, entirely new challenges emerge. Growth sounds exciting. But growth without infrastructure creates chaos very quickly. And high-performing organizations understand that scaling successfully requires discipline.

  • Episode 184: Busy, Burned Out, and Barely Profitable? What Top Groups Do Differently - Part 1
    Jun 2 2026

    Send us Fan Mail

Why do some medical groups thrive while others constantly struggle — even in the same market?

Have you ever looked at two practices in the same city, with similar physicians, similar patient populations, and similar payer contracts… and wondered why one group seems calm, strategic, financially healthy, and growing — while the other feels stressed, chaotic, reactive, and constantly fighting fires?

That difference is rarely luck. High-performing medical groups think differently. They make decisions differently. They build systems differently. They lead differently. They invest differently. And perhaps most importantly, they understand that success in healthcare today requires far more than simply being clinically excellent.

Because let’s be honest — almost every physician group is working hard. That’s not the differentiator anymore. The differentiator is operational intelligence. The differentiator is leadership. The differentiator is culture. The differentiator is the ability to think strategically instead of reactively.

And today, we’re going to talk about what high-performing medical groups actually do differently. Not from a theoretical textbook perspective. From the real-world trenches of healthcare operations. Because thriving organizations don’t happen accidentally. They are built intentionally.

    May 26 2026

    Send us Fan Mail

What if your organization’s biggest revenue problem has nothing to do with payer contracts?

Think about that for a moment.

Because in healthcare, we spend enormous amounts of time obsessing over payer negotiations, denials, collections, reimbursement schedules, staffing ratios, labor costs, and operational efficiency. Entire leadership meetings are devoted to squeezing another one or two percent out of contracts.

But what if the revenue problem starts long before the claim is ever submitted? What if the biggest leak in your organization is happening quietly, invisibly, every single day inside the clinic? Today we’re uncovering the million-dollar problem nobody is talking about: physician undercoding.

In this episode, I sit down with Saurabh Gupta, MD — physician, healthcare technology executive, and Founder & CEO of CorMetrix — for a timely conversation about the explosion of healthcare data and how artificial intelligence can help physicians and healthcare organizations make sense of it.

With decades of experience spanning clinical medicine, enterprise healthcare technology, data strategy, and innovation, Dr. Gupta brings a unique perspective to the intersection of medicine, AI, and operational intelligence. We explore how healthcare leaders can move beyond data overload to uncover meaningful insights that improve decision-making, efficiency, compliance, and ultimately patient care. • Episode 181: The Power of Private Practice: Why Independence, Community, and Connection Still Matter
    May 12 2026

    Send us Fan Mail

Today I want to talk about something that is deeply important to me, and I suspect deeply important to many of you listening today — the future of independent private practice, and why community matters now more than ever.

This past weekend, we gathered in Portland, Oregon for the Caduceus Masquerade Ball, a celebration of independent medicine and the people who dedicate their lives to caring for our communities. And honestly, it was extraordinary.

The room was filled with physicians, practice administrators, healthcare leaders, consultants, and supporters of private practice. There was elegance and laughter and music and conversation. People who had emailed each other for years finally met face to face. Specialists met referring physicians they had never actually met in person despite sharing patients for years. Colleagues reconnected. New relationships formed.

And perhaps most importantly, there was this unmistakable feeling in the room — hope.

Not denial about the challenges in healthcare. Not pretending everything is easy. But hope grounded in the realization that we are still stronger together than we are apart.

What made the evening even more meaningful was that the Caduceus Ball was also a fundraiser supporting three local nonprofit organizations that serve physicians, healthcare teams, and medical practices in our community. So everyone attending wasn’t just celebrating — they were also contributing to causes that strengthen healthcare locally. There is something profoundly uplifting about gathering together not only for ourselves, but also in service of others. That sense of generosity and shared purpose changes the energy in a room.

And it reminded me of something important:

Private practice is not simply a business model.

    It is a community model.

  • Episode 180: Drowning in Reports, But Still Guessing? How to Turn Practice Data Into Real Decisions
    May 5 2026

    Send us Fan Mail

Today we’re talking about something that is hiding in plain sight in almost every practice I work with—and that is your data. Because here’s the truth: most practices are sitting on a goldmine of data inside their EMR and practice management system… but very few are actually using it to make better business decisions.

You’ve got reports. You’ve got dashboards. You’ve got numbers. But if you’re like most physicians and practice leaders, you’re still asking: what does this actually mean, what am I supposed to do with this, and why do I feel like I’m drowning in information but still guessing on decisions?

If that sounds familiar, you’re not alone—and you’re not behind. You just haven’t been given the right framework yet. So today, I’m going to walk you through what business intelligence actually means in a medical practice, why most practices don’t fully use their data, why having the right analyst can completely change your trajectory, and three very practical ways you can start using your data to make better decisions starting now.

  • Episode 179: Stop Wasting Conferences: How to Get Real Value from Every Event
    Apr 28 2026

    Send us Fan Mail

It’s conference season—and calendars are filling up fast. Flights are getting booked, hotels are getting reserved, registration fees are getting approved, and calendars are getting blocked off for a few days away from the practice. And if you’re like most physicians and practice leaders, there’s a mix of emotions. Maybe a little excitement. Maybe a little overwhelm. Maybe even a quiet question in the back of your mind…

    Is this actually worth it?

Because when you really step back and look at it, attending a conference is not a small decision. It’s not just the registration fee. It’s the travel. The hotel. The meals. The time away from patients. The disruption to your team. The mental energy. This is not a $2,000 decision. For most people, this is a $5,000 to $10,000 investment… sometimes more.

And yet, what I see over and over again is that people treat conferences almost like a passive experience. You show up. You sit in sessions. You take a few notes. You maybe talk to a couple of people. And then you come home… and within a week or two, it’s like it never happened. Nothing really changes.

So today, I want to challenge that.
